ELC School Holiday Program booking form

Dear families,

The ELC will be open for a School Holiday Program between 14-24 April 2020.

The holiday program is open to current ELC and Kinder children and siblings in Year One, with priorty to ELC families first. Spaces are limited, please book early to avoid missing out.

Local Walks Consent: I give consent for my child to participate in the excursion for local walks and agree to delegate my authority to the staff involved. Such educators may take appropriate disciplinary action in accordance with school policy to ensure the safety, well-being and successful conduct of the students as a group or individually in the abovementioned activities. I also authorise the educators to provide First-aid and to obtain necessary medical assistance should an accident occur. I undertake to pay all medical expenses incurred on behalf of the above student, including medical treatment, ambulance transfers and prescribed medications. I submit the medical information provided on the Medical Consent Form and will advise updates if medical conditions change. *
