Please complete this form if your child is has an enrolment pending at another school or is not returning to Holy Trintiy for the 2022 School Year. This information is required as we being the planning stage of the 2022 school year during these school holidays.

Families with Year 6 students do not need to complete this form as we know that they will not be returning.
